Request forms and follow-ups
Let anyone on staff submit a request, and create follow-ups for people.
Request forms
A request form lets anyone who can see the workflow submit a new item without opening the board, like an event request or a facilities request. Each submission becomes a new item.
Turn on the request form
In the workflow’s settings, open Request form and turn on Take requests. Then choose:
- Form title and Intro text.
- Fields on the form. The title is always asked. The rest follow your Fields list. Fields marked On form appear, and Required on the request form makes them required.
- New requests start in: the stage new requests land in.
- “Read this first” doc: a doc people must read before they submit.
- Thank-you message: what people see after they submit.
Select Open the form to try it or Copy link to share it.
Submit a request
- Open the form link.
- If there’s a “read this first” doc, select Read and go through it.
- Fill in the title and fields, then submit.
You’ll see Submitted, with View it and Submit another. You’ll be notified as it moves forward, and you can follow it in My work → Created by me.
Follow-ups
A follow-up is an item linked to a person, like calling a first-time guest. You can create them from People:
- Open Community → People.
- Select Create follow-up… on a person, or Create a follow-up for everyone on this page at the top.
- Choose the Workflow, how to Assign (Don’t assign yet, To one person, or Take turns between people), the Due date, and an optional Note.
Each follow-up is linked to the person’s profile, and their profile shows the workflows they’re in.
Automations can create follow-ups for you, for example whenever someone fills out a guest form. See Automations.
